Sending us a referral does not create or confirm a booking. We are not able to book appointments on a patient's behalf. Every E-ADHD appointment must be booked directly by the client through our secure online portal. If you have referred a patient to us, please also ask them to complete their own booking online, as our team cannot action a referral as an appointment request. Client booking portal.
Our psychiatrist does not diagnose ADHD
Diagnosis and medication are two separate appointments with two different clinicians. Our Clinical Psychologist completes the ADHD diagnosis. Our Psychiatrist completes the medication assessment and initiation, where clinically appropriate. If you do not yet have a diagnostic report, please book an ADHD initial diagnosis with our Clinical Psychologist.
Ongoing management with our GP with a Special Interest
If you already have a formal ADHD diagnosis and a medication plan from a psychiatrist, book the 20 minute follow up and monitoring appointment with our GP with a Special Interest in ADHD. This appointment is for ongoing management and script renewal only. Our GP with a Special Interest does not diagnose ADHD, so a diagnosis cannot be made in this appointment. What to have ready before your appointment with Dr Lawrence Kozlowski
If you already have an ADHD diagnostic report, your next step is the 45 minute First Appointment for medication appointment and initiation with our Consultant Psychiatrist, Dr Lawrence Kozlowski, shown in the booking portal as "Initial Medication Assessment and Follow Up". Please have three items ready to upload: a copy of your initial ADHD diagnostic report, a current GP referral valid for 12 months, and baseline cardiovascular measurements from your GP, being blood pressure, heart rate, height and weight. If you are eligible, you may receive a Medicare rebate of up to $265.60. Eligibility depends on your previous use of Medicare rebates and whether you have an existing Mental Health Care Plan, so we recommend confirming this with your referring GP before booking. What to have ready before your child's paediatrician appointment
For children and young people aged 6 to 25, medication appointment and initiation is completed by our Paediatrician. If your child already has an ADHD diagnostic report, please have three items ready to upload before the appointment: a copy of the initial ADHD diagnostic report, a current GP or paediatric referral valid for 12 months, and baseline measurements from your GP, being blood pressure, heart rate, height and weight. Medicare rebates may be available depending on your referral and your previous use of Medicare rebates, so we recommend confirming this with your referring GP before booking. A written diagnostic report from a psychologist, clinical psychologist, paediatrician, or psychiatrist confirming the ADHD diagnosis (DSM-5 criteria). This report is the foundation of your child's medication assessment.
If your child has been assessed by a psychologist, please provide their report or letter outlining assessment findings, cognitive testing results, and any co-occurring conditions (e.g. anxiety, learning difficulties, ASD).
A current referral from your child's regular GP addressed to the E-ADHD Consultant Physician (Paediatrician or Psychiatrist). This is required to access Medicare rebates and allows us to coordinate shared care with your GP. If you cannot access a referral from a regular GP, you can schedule with an E-ADHD GP-Special Interest, for a referral.

A written diagnostic report from a psychologist, clinical psychologist, psychiatrist, or paediatrician confirming your ADHD diagnosis against DSM-5 criteria. If you were diagnosed as a child, please bring historical documentation where available.
If you've been assessed by a psychologist, please provide their report outlining cognitive testing, rating scale results (e.g. ASRS, DIVA-5, CAARS), and any co-occurring conditions such as anxiety, depression, autism, or sleep disorders.
A current referral from your regular GP addressed to a Consultant Physician or Psychiatrist. If you are seeing an E-ADHD GP-Special Interest, you do not need a GP referral.
Baseline cardiovascular measurements from your GP taken within the last 3 months. If you're over 40 or have a personal or family history of heart disease, your clinician may request an ECG before initiating psychostimulant medication.
